Overview

Cerebrolysin is a standardized preparation of low-molecular-weight peptides and free amino acids obtained from purified pig brain protein. It is designed to mimic the activity of the brain's own neurotrophic factors such as BDNF, GDNF and NGF, supporting neuron survival, synaptic plasticity and repair after injury. It is given as a course of intramuscular or intravenous injections rather than as a daily ongoing therapy. Although widely prescribed in Europe, Russia and Asia for ischemic stroke, vascular and Alzheimer's dementia, and traumatic brain injury, it has never been approved in the United States. Trial results are mixed: some studies show improved early functional recovery, while pooled analyses show no mortality benefit. Its porcine origin means allergy screening matters.

Research notes

  • The CARS trials (2016) suggested improved early motor recovery when started within 72 hours of stroke, but larger trials such as CASTA showed no overall benefit.
  • Cochrane 2023 (7 RCTs, 1,773 participants): probably no effect on all-cause death in acute ischemic stroke; possible increase in non-fatal serious adverse events.
  • Meta-analyses in Alzheimer's and vascular dementia report small improvements in global function and cognition over 4–6 months; trial quality is moderate.
  • Use for cognitive enhancement in healthy adults has not been studied in controlled trials.
