Laeeq M.D. Concierge Practice (“Laeeq M.D.”, “we”) is the medical practice of Dr. Laeeq Ahmed Butt, M.D., MBA, located at 1920 Association Drive, Suite 509, Reston, VA 20191. This policy covers this website and the patient portal.
Information we collect
- Information you give us — contact forms, newsletter sign-ups, portal registration, intake forms, consultation and refill requests, and secure messages.
- Consultation requests — when you request a consultation through our form or website assistant, your contact details and request are sent to the practice’s email and our patient-relationship (CRM) system so we can contact you.
- Website assistant — questions you type into the website assistant are processed by an AI service to generate educational answers. Please don’t enter detailed medical information there.
- Technical information — basic server logs (IP address, browser type, pages requested) used for security and reliability.
- Cookies and local storage — essential items that keep you signed in and remember your cookie choice. We do not use advertising cookies.
How we use information
- To respond to inquiries, schedule visits and provide medical care.
- To operate the patient portal, including messages, requests, appointments and lab results.
- To send the newsletter if you subscribe (you can unsubscribe at any time).
- To keep the website secure and working properly.
We do not sell personal information, and we do not share it for advertising.
Health information
Health information you share with us as a patient is handled as protected health information under applicable federal and state law, including HIPAA. Our Notice of Privacy Practices, provided at your first visit and available on request, explains your rights over medical records. Please do not send detailed medical information through the public contact form — use the patient portal instead.
Service providers
We use trusted vendors to host the website, run the patient portal database and authentication, deliver email, manage patient communications and scheduling, and power the website assistant. They may process information only on our instructions and are required to protect it.
Data security and retention
Portal data is encrypted in transit and access is restricted by account and role. Medical records are retained for the periods required by law; other information is kept only as long as needed for the purposes above.
Your choices and rights
You may request access to, correction of, or deletion of personal information we hold (subject to medical-record retention laws), opt out of the newsletter, and choose essential-only cookies. Residents of some states, including California and Virginia, have additional rights under state privacy laws.
Children
This website and our services are intended for adults 18 and older.
Changes
