Overview
Tirzepatide is a single peptide that activates two incretin receptors: GIP and GLP-1. Combining both signals enhances insulin release, suppresses appetite and slows stomach emptying, and GIP may further improve fat-tissue metabolism and tolerability. A fatty-acid chain gives it a roughly five-day half-life for weekly dosing. In the SURMOUNT trials, the highest dose produced about 21% average weight loss over 72 weeks, and a head-to-head study found it outperformed semaglutide. It is approved for type 2 diabetes, obesity and obstructive sleep apnea. As with all incretin drugs, protein intake, resistance training and a plan for maintenance are essential.

- Cardiovascular outcome data are still emerging; weight regain occurs after discontinuation (SURMOUNT-4).
